Support Practitioners 30 Hours – Intensive Outreach Service

Bath Mind is seeking a Support Practitioner to provide flexible support to clients with ill mental health living in their own homes across B&NES, with the support of Avon and Wiltshire Mental Health Partnership (AWP).

About the role

Hours: 30 per week

Holiday: 25 day’s holiday per year + Public and Bank Holidays (pro rata)

Salary Package: £18,548.40

About the team

The Intensive Outreach Service works in collaboration with AWP providing support to clients leaving psychiatric care (step-down) or to prevent the need for hospital admission (step-up) by delivering intensive mental health support to people in their own homes.

You will offer daily support to meet the needs of the clients.

All applicants must have access to a car and have a full drivers licence.
