Join our Intensive Outreach Service as a Support Practitioner
Bath Mind is seeking a full time Support Practitioner to provide flexible support to clients with ill mental health living in their own homes across B&NES, with the support of Avon and Wiltshire Mental Health Partnership (AWP).
About the role
Hours: 37.5 hours per week Monday – Saturday.
Holiday: 25 days + bank holidays pro-rata
Salary Package: £23,185.50
About the team
The Intensive Outreach Service works in collaboration with AWP providing support to clients leaving psychiatric care (step-down) or to prevent the need for hospital admission (step-up) by delivering intensive mental health support to people in their own homes.
You will offer daily support to meet the needs of the clients.
All applicants must have access to a car and have a full drivers licence.
