The Crisis Resolution & Home Treatment Team	01225 825 328	The service is a team of mental health practitioners providing an emergency assessment and home treatment service for people living in the Bath and North East Somerset area experiencing a mental health crisis and who would otherwise need admission to hospital. The service is available to adults aged 16-65 years and accessible through a referral from your GP or Community Mental Health Team.
Community Mental Health Teams - Bath	01225 731 631 Mon-Fri 9am-5pm	These teams are based at NHS House, Newbridge Hill, Bath The Community Mental Health Teams (CMHTs) provide assessment and treatment for people living in the community who are experiencing severe mental health problems or difficulties. The aim is to enable you to continue to live in the community, according to your individual needs and wishes. CMHTs also offer support for carers, assessing their needs to help them get the help and assistance they need. Referral is through a health or social care professional.
