Community Support

Our B&NES accredited mental health support service offers practical assistance to people within their own accommodation throughout the county to help people live an independent and fulfilling life.

The support can offer:

  • Recovery and adjusting back from a period of hospitalisation
  • Sustaining a tenancy (paperwork, budgeting, helping with your environment)
  • Health and wellbeing support
  • Help to budget
  • Support to deter loneliness
  • Communication to others
  • Support with meetings
  • Developing social networks
  • Accessing community facilities (leisure, work, education)
  • Confidence building

How to refer...

You can be referred to our service through health services, your GP, housing departments, families, voluntary agencies or yourself using our online referral form.

In the first instance we carry out a face to face assessment of your needs and goals - this is to make sure we can properly support you.

All service users are encouraged to be actively involved in designing their own care package. A plan of support will be developed and agreed by you and reviewed regularly to make sure we are on track to reach the desired outcomes.

Your support hours can be flexible to suit your needs throughout the week. Please note we do not currently offer an overnight service.

Our staff...

Our staff are experienced and trained in issues surrounding mental health conditions, health and safety, safeguarding and the delivery of person centred support.

All staff are employed only after an enhanced Disclosure and Barring Service (DBS) check and professional referrences have been obtained.

Our fees...

The cost of the service is 20* per hour. A breakdown of how this cost is calculated is available.

Our service can be funded through health services or through an individuals personal budget.
